Meta-analysis of effectiveness and safety of D2 plus para-aortic lymphadenectomy for resectable gastric cancer.

چکیده
Study selection Randomised controlled trials (RCTs) that compared D2 plus para-aortic lymphadenectomy (including dissection of middle para-aortic lymph nodes, 16a2, 16b1) with standard D2 lymphadenectomy in patients with resectable, biopsydiagnosed, gastric adenocarcinoma were eligible for inclusion in the review. Studies had to report more than one of the specified outcome measures. The primary outcome of interest was five-year overall survival. Secondary outcomes were in-hospital or postoperative 30-day mortality and postoperative morbidity (including anastomotic leakage, pancreatic fistula, abdominal abscess, postoperative bleeding, obstruction or ileus, wound infection and cardiocirculatory and pulmonary complications associated with the procedure).

Functional outcomes after extended surgery for gastric cancer (Br J Surg 2011; 98: 239-245).
BACKGROUND Extended gastrectomy with para-aortic nodal dissection (PAND) or thorough dissection of mediastinal nodes using a left thoracoabdominal (LTA) approach is an alternative to D2 lymphadenectomy, with variable postoperative results. METHODS Two randomized controlled trials have been conducted to compare D2 lymphadenectomy alone (263 patients) versus D2 lymphadenectomy plus PAND (260), ...

Background: Surgical staging is the standard treatment of ovarian cancer. Pelvic and para-aortic lymphadenectomy is the important part of the surgery. The aim of this study was to evaluate the effect of para aortic lymph node dissection in early stage of patients with ovarian cancer. Methods: This descriptive cross-sectional cohort study was performed on all stage I of ovarian cancer pati...
متن کاملSelective lymphadenectomy of para-aortic lymph nodes for advanced gastric cancer.
The Japanese randomized trial comparing standard D2 with D2 plus additional para-aortic lymph node (PAN) dissection for advanced gastric cancer (JCOG study 9501) did not demonstrate any difference in survival between the two groups. It is unknown whether there is any prognostic benefit in dissection for subgroups of PAN. Non-inferiority in survival of the patients with PAN metastasis to the pat...
متن کاملAn evidence-based medicine review of lymphadenectomy extent for gastric cancer.
BACKGROUND Several studies in the literature have investigated the possible role of the extent of lymphadenectomy in gastric cancer treatment failure. The current study attempted to determine the effectiveness and safety of lymphadenectomy with gastrectomy for the treatment of gastric cancer. METHODS Randomized controlled trials (RCTs) were identified by means of MEDLINE, EMBASE, Cochrane Con...
متن کاملEvaluation of rational extent lymphadenectomy for local advanced gastric cancer
Based upon studies from randomized clinical trials, the extended (D2) lymph node dissection is now recommended as a standard procedure for local advanced gastric cancer worldwide. However, the rational extent lymphadenectomy for local advanced gastric cancer has remained a topic of debate in the past decades. Due to the limitation of low metastatic rate in para-aortic nodes (PAN) in JCOG9501, t...
